one hundred eighteen

it was early in the morning when gwen stood in the lobby from the hospital writing something in a chart.


owen asks the staff to meet up for an announcement.


gwen looked up when she saw mark approaching her.


"i brought hailey to day care." he informs her.


"thank you." gwen smiles.


"morning guys." callie greets them.


"hi." the couple replies.


as derek walked up to them callie grabbed his arm.


"uh, an acoustic neuroma?" callie asks the neurosurgeon outraged.


"oh, boy." derek sighs.


"a long, difficult surgery with a stupid-high post-excision mortality/morbidity?" callie asks.


"wait, that's what you want to start our hand with?" mark asks with raised brows.


"it's my hand." derek reminds them. "and thanks for making it sound so fun."


"you can't try something with a little margin for error?" callie asks.


"i was supposed to operate on jimmy months ago, but then the plane crashed. it has to be today." derek says.


"well, if you can do this, i guess my hand can do anything." mark states.


"still my hand." derek glared at mark.


"okay, folks. let's just start." owen spokes up gaining everyone's attention. "as many of you know, seattle grace-mercy west has suffered a financial setback. the administration is working hard to keep the impact of that setback as small as possible. to that end, we're gonna have to initiate some cost-cutting measures. the board has hired a physician advisor to help."


"what's a physician advisor?" mark asks confused putting his hand on gwen's lower back.


"well, she consults on maximizing efficiency, streamlining processes, assessing where we can trim out budgets." owen explains.


"who to fire and when to fire them?" bailey jokingly asks making the crowd laugh.


"she's here to advise and to help." owen tells them. "i was hoping to introduce her in person, but she seems to be late."


"oh, on her first day? not very efficient." gwen comments making the staff laugh again.


"you can expect to see her in the halls, in your labs, in the o.r." owen continues ignoring gwen's comment.


"so we're gonna have an accountant following us around in the o.r.'s?" derek skepticaly asks.


"she's not an accountant. she's a trained surgeon." owen corrects him. "frankly, she is here to try to help us. thank you for your cooperation."


then owen left as everyone went on doing the things they need to do.


"hey. can i get a minute? it's urgent." gwen turns to mark.


"yeah." mark nods. "for what?"


"you will see." gwen took his hand pushing him into a on call room.


"oh, this is urgent?" mark asks as gwen pushes him on the bed to straddle him.


"yes." gwen replies pushing down his coat. "it's the stupid pregnancy hormones. it makes me want to have sex all the time, urgently."


"i like the stupid hormones." mark smirks as she kissed him.


but then gwen's pager ringed.


"oh, no." she sighs.


"what?" mark asks continue kissing her neck.


"oh, i have a consult." gwen explains.


"what?" mark asks as gwen suddenly started to cry. "are you crying?"


"no. yes." gwen nods as mark cuppes her face. "i cry everytime i get mad."


"you're not... you're mad?" mark asks confused.


"yes, i'm mad, because i can't have sex, because i have this stupid consult." gwen cries.


"oh, but please don't cry because of it." mark chuckles whipping her tears away.


gwen groaned angry when her pager went off again.


"uh god, i'm coming, okay?" gwen groans as she stood up leaving the room.


"i like these hormones." mark mumbles putting his coats back on.

gwen got paged by cristina to assist on a patient with arizona.


she walked into the room to see a little girl and her mom and the two surgeons already there.


cristina asked gwen to look over the scans on the monitor.


"everyone at namboze was so worried when we heard the program might move to u.c.l.a, myself included." the patients mother, makena, tells them. "you make the kids feel so at home here."


"well, we were never gonna let that happen." arizona assures her. "no, i was away, but now i'm back. and as long as dr.karev and i are here, then the program stays here." arizona adds.


"okay, um, nyah's pericarditis is quite severe." gwen spokes up after shortly studying the images. "the pericardial sac around her heart is inflamed and starting to adhere to the heart itself. so i'll perform what's called pericardial stripping. so i'll remove the sac, and her heart will start beating on its own again."


"what shall i tell her?" makena asks the doctors nodding to nyah who couldn't understand english.


"oh, tell her we're gonna fix her heart, and she'll breathe much better." arizona replies.


then makena started to speak in an african dialect.


gwen and cristina walked out of the room together down the hall.


"nice program. shame to see it go." cristina says making gwen look at her.


"yeah. wait. what?" gwen asks confused.


"we're getting a visit from the efficiency fairy. you think she's not gonna make this disappear?" cristina asks her former attending.


"we just got it back." gwen says.


"she's trimming fat. they're flying kids across the world for a pro bono surgery. that's pretty fatty." cristina tells her.


"yeah, they're orphans." gwen reminds her.


"well, now you're just making my point." cristina says walking away.

"the pericardium is free." cristina announces as they stood in the o.r. "you should see an increase in, uh, cardiac function."


"it's weird. heart's floppy." gwen states. "still not the way a kid's heart should look."


"mmm. could be endocarditis." cristina looks over to gwen who nods. "i'll do a biopsy. biopsy needle, please."


"unless its... oh, my god. it couldn't, could it?" gwen suddenly gasps both cardio surgeons looking with wide eyes at each other. "could this be endomyocardial fibrosis?"


"i've only read about it. it's rare, subtropical." cristina adds. "could this be that?"


"maybe." gwen replies. "remember we had that kid with pentalogy of cantrell?"


"this program is a medical freak show goldmine." cristina says. "they can't cut it."


"nobody is cutting it." gwen reminds her. "arizona, tell her to stop saying that."


"i'm sorry. what?" arizona asks groaning.


"you all right?" gwen asks concerned.


"yeah." arizona nods.


"okay, um, take that to path." cristina orders the nurse. "stat, please."


"oh, you know what? i'll take it." arizona offers. "gwen, you take my place."


"but the nurse can take it." gwen reminds her.


"no, i'd rather take it myself." arizona insists.


gwen watched her confused as she left the o.r.

after nyah's surgery gwen talked to her mother when dr.cahill joined them.


gwen explaines how the surgery went when arizona walks up to them.


"dr.robbins." cahill greets her. "everything, uh, work out all right?"


"yeah, i'm sorry that you had to see that. it was a personal thing." arizona explains.


"you were having a moment. we all have 'em. it just sucks when they happen at work, right?" cahill asks.


"i was, uh, just telling dr.cahill how well nyah's surgery went." gwen informs arizona. "and that you let me finish the surgery on my own. thanks again."


"that shouldn't be that impressive for an attending." cahill says.


"yeah, but actually i'm an cardio surgeon." gwen tells cahill more sternly already annoyed by her.


"well, i will be observing nyah's second surgery tomorrow." cahill informs them. "so will that be you or dr.shepherd?"


"oh, no, me, of course." arizona replies.


"yeah, with me and my cardio fellon, dr.yang." "gwen adds.


"all right. i will see you there." cahill then walks away.


"what's going on?" gwen asks her ex when cahill was gone.


"nothing. i'm fine." arizona replies. "i'm sorry about this morning. are we all set for tomorrow?"


"uh, 8:30, o.r. two." gwen tells her. "look, if you want to sit it out, yang and i can handle it. maybe, you know, you should just take it easy."


"are you finished?" arizona cuts her off.


"i just want everything to go well." gwen retorts.


"then i'll see you tomorrow morning." arizona said before leaving.

gwen stood in the o.r. the next day on nyah's second surgery with cristina and arizona.


everything went fine except the fact that cahill and owen watched the whole surgery.


suddenly arizona stepped back from the table knocking over some surgical tools.


"what happened?" cristina asks.


"you need me to step in?" gwen turns to arizona.


"no. no." arizona shokes her head as she exhales deeply.


"robbins?" owen calls her over the intercom. "try to picture the waves."


"i'm trying." arizona tells him.


"okay, really, what is happening?" cristina repeats her question.


"just calm the waves." owen instructs arizona.


"these are tsunami-force waves, owen." arizona informs him.


"look, you should go. i'll take over." gwen offers.


"all right. no one is moving." cristina sternly says. "i'm stripping the endocardium from the papillary muscle. i screw this up, i'll have to replace the mitral valve, so don't move."


"i can switch out-" gwen offers again.


"just shut up." arizona spits. "i'm at the beach."

after another few minutes of operating arizona turned to gwen.


"i have to switch, gwen." she tells her.


"no one is switching." cristina repeats. "you're holding the mitral valve, so don't move."


"what's wrong? are you sick?" gwen asks arizona concerned.


"gwen, take that scalpel... and stab me in the foot." arizona suddenly says.


"what? what are you doing?" cristina asks with wide eyes.


"my prosthetic. stab me." arizona orders her ex.


"what's wrong with you?" gwen asks.


"dr.shepherd, i am the chief of surgery." owen yells over the intercom making them look up. "this is an order. stab dr.robbins in the foot right now."


"gwen." arizona calls her once again.


"arizona? gwen!" cristina yells her name when gwen took the scalpel stabbing arizona right in the foot.


they all watched as arizona looked down at her shoes slightly smilling.


"that's better." she smiles.

after the surgery gwen and cristina went to nyah's mom telling her how the surgery went.


"her heart is, uh, pumping like a champ." gwen informs the relieved mother.


"thank you, both of you." makena tells them.


"what else do you have over there?" cristina suddenly asks.


"i'm sorry?" makena furrows her brows.


"uh... sick... orphan... children? kids with heart problems?" cristina asks.


gwen cleared her throat. "she means, she's glad the program is up and running again. both of us are."

after a full day of work gwen had to go to this meeting cahill announced in the conference room.


she walked with a cup of coffee into the room sitting down next to mark who took her hand giving it a kiss.


"all right, welcome. welcome, everyone." cahill greets them. "thank you so much for coming. it has been a privilege to watch you work, and i'm so impressed with what i've seen. dr.karev's pediatric african exchange program-"


"you cannot cut that program." cristina suddenly jumped up making the crowd mumur. "i'm sorry, but these children, these ...little orphans... we are the only hope they have. many of them have rare disorders that most western doctors rarely have the opportunity to treat."


gwen and meredith exchanged an amused glare.


"and if you stop the program, i will never get that chance again." cristina adds earning a few looks. "they. they will never get that chance, the precious children." cristina corrects.


"i have no intention of cutting it." cahill infroms her. "it's a no-brainer from a pr standpoint. that kind of positive visibility is gold. however, there will have to be some cuts. and i'm going to suggest one that is going to seem suprising at first, but that adresses fiduciary concerns with nominal impact on the core facility and the human and physical resources."


"oh. i'm sorry. what?" jackson asks.


"uh, yeah. i'm out." bailey agrees.


"i'm proposing that we close your e.r." cahill announces making the room gasps and talk.


"we're a level one trauma center." owen reminds her.


"we're a hospital." webber adds.


"in the last three days, your e.r went from being in shambles to peak efficiency." cahill explains.


"well, then why would you propose cutting it?" april asks outraged.


"this allowed me to see that in either case, it's not bringing in the money." cahill tells them.


"it's bringing in the patients." owen reminds her.


"over half of whom are non-emergent, underinsured, and belong in a primary care physician's office." cahill says. "you have surgical interns in there doing chest exams. it's hemorrhaging resources. look, i know these cuts are going to be painful. but understand that i am trying to keep your hospital doors open."


"this is ridiculous." mark spokes up. "you can't close the e.r.-"


"do you have a better idea?" bailey cuts him off. "because the way i see it, this woman is trying to keep the hospital from closing, in which case there'd be no patients, we'd have no jobs, and some of us don't have a few million to fall back on. so unless you have a better idea, why don't you just sit there and listen to what she has to say?"


gwen looked at her with wide eyes and mouth agape.


she had to controll herself to not just slap her in the face for saying that.


it wasn't there fault that the plane crashed.


callie and her exchanged glares looking both pissed.

after finally getting home gwen stood in the bedroom looking through her clothes to find some nightwear.


"this is so stupid." she says mad. "we're a level 1 trauma center. this woman's an idiot."


"well, and a little bitchy." mark adds as he walks out of the bathroom.


gwen sat down on the bed mark next to her.


"bailey had no business calling you out like that." gwen says.


"she's nervous. everybody's nervous." mark sighs. "everybody should be nervous."


"it's not our fault." gwen quietly says taking his hand. "is it?"


"it feels like it is." mark confesses.


"i almost died and know i'm blameworthy for everyone losing their job." gwen sobs.


"you cry again because you're mad?" mark asks slightly amused.


"yeah. stop laughing." gwen says but couldn't keep her smile off her face.


"let's try and go to sleep." mark suggests kissing her temple.


"i don't think i can. i'm too pissed." gwen says.


"then go to sleep anyway. you're pregnant you need your sleep." mark says already under his blanket eyes closed.
